Recently I have rescued a mbu puffer from a friend's son who was not taking proper care of the fish. When I picked him up he had a sore on his left side when looking at him head on, which has begun to heal quickly with proper dosage of melafix. I have a 55 gallon cycled hospital aquarium with the following water readings:

pH 7.6
Ammonia 0ppm
Nitrite 0ppm
Nitrate 40ppm

Temp @ 77F
Marineland C-350 Filtration with 2 airstones going

He has suffered from ich since I have brought him home (I suspect the 3 hour long car ride in a cooler on air, which I felt horrible for:cry:), which he has recovered from after keeping the temperature at 80-84 with a slight amount of salt for the duration of 10 days.
He is very lethargic in the mornings until around noon when he starts moving about, during this time he will sit with his tail clamped up, and almost look like a lump on the bottom.
If an additional power head is on he will begin swimming around also we add a dosage of melafix he will also swim around and become much more active.
Now the uphill battle is attempting to clear up the ongoing redness and cloudiness in his eyes. It almost seems that he has cataracts and I have read this could have been due to the very poor water quality that he was retained in for such a long period of time.
My friend's family had attempted everything they could keeping him in a 55 gallon with danios and tetras, but needless to say they gave up on the cause.

My goal is to get him in a better situation so I can begin cycling a second 150 tank to move him into. (I currently have a 150 gallon housing an arrowana, TSN, and clown knife)
He seems to be in good spirits despite the fact that he must have little to no vision in the afternoon and evening. His diet at the moment is consisting of squid, krill, a mussel once a week, I have tried live snails with little luck.

Anything would help because I would love to see this little guy be the highlight of my home.
I do not know very much about these fish and after a couple weeks of borrowing library books I figured that maybe a forum would be the way to go.

Does not look that bad to me. Just keep doing what you are doing, keep the water super clean and a healthy diet. Things shuld clear up in time.

The pupil is always yellow, that is not from the camera flash. He moves the eyes around quite frequently but if you are cleaning the tank you have to push him around like he is almost blind.

Has he been treated for internal parasites? I would do it if it is even slightly possible that he wasn't treated as almost every puffer comes with them.

That is a great point! I do not know much about puffers, and this may be an answer after reading more about puffer parasites. Any recommended medication out of the various ones I have read about?

That eye is useless. How about the other eye? One of the photos you provided here seem to show that it is different and not having a yellow pupil. There is no clear shot of the other side of your puffer. Can you add another pic to show the other side?

It's exactly the same. :/ He normally sits with his right side facing the tank, and it's really hard to get him to turn around.. Since he only really swims up and down.
