roots are rotting off, plants being uprooted, stems snapping in half-- HELP!

somebody is uprooting every plant the minute i leave the room. even my amazon sword with GIANT root network, 4" of solid thick root planted in 3" of flourite substrate is being uprooted. its gotten really out of hand. ive hand these plants for 2 days and everything but the hornwort is dying. i have 260 watts of 4 bulb t5ho on my 100gal, no co2 on 8 hours a day just bought seachem trace and excel been dosing 5ml past day. my ludwigia stems at the base well lets just say they dont have a base stem anymore its completely decayed. none of them have roots but they used to. my echinodorous both are constantly uprooted despite having a gigantic root network. one of them looks fantastic and the other looks rancid! the one has almost completely melted (this one had lots of roots, now its down to about 1-2" of it). my plants look HORRIBLE! the wisteria stems dont last 10 minutes planted in the substrate! ive got about 30-40 or more ripped leaves i had to take out and throw away! some of them have little tiny bit of roots most dont though. the hornwort isnt thriving but its 'surviving'. ive replanted the crypts about 8 or 9 times but they look fine besides the stems have turned red like its got a bad rash. the crypts have big root systems too. then theirs this one plant i dont know what it is i do know its a stem plant and its also been torn into bits but it looks like some of the broken off pieces are developing roots and growing tiny leaves. the only 'surviving' non-dead plants are floating. i come in the room and somebody litterally took a shovel and just dug it up. theirs always a big hole next to the plants and a big ring of gravel that used to cover the hole. my bala shark has been nipping everything! its either hes eating it or its melting and im starting to think the echinodorus is melting and hes eating the rest. i did check thoroughly for snails and i have not found a single one having spent hours planting and trimming and planting and trimming. and on top of that, my newly acquired 5" bristolnose wont touch green algae! its not thread algae guys i know what green algae looks like and i thought pleco's love it!? i have like 20 pieces of lava rock and driftwood and my lights are blindingly powerful. i tried to move him off the driftwood to see if he'd go to the glass and the son of a gun BIT ME! so before i fouled my tank with blood i had to run down stairs with a towel grab a bandaid and cover it and then i went back upstairs and GUESS WHAT the plants have all been uprooted.... so then i went back in the tank this time with my left hand (im right-handed using my left hand is kinda hard) and tried to replant them and right in the corner of my eye... AHA! theres that damn giraffe catfish sucking in the gravel like a vaccumm and spitting it out and the next thing i know the last plant that was surviving in the substrate is again floating... so im back to trimming the wisteria which is all over the place and ive got like an ounce of plant matter in a little baggy (plants are light thats like alot of plants!) and im going down to the trash to clean up the mess cuz im finally giving up and i come back up and BAM! its uprooted again! and to top it all off this is the second time im writing this thread as the first time i accidentally hit control-delete!
HELP ME!!!!! *pleading*
 
*light bulb* could the brittle stems be a nutrient deficiency because I have really high lighting and I'm dosing 15% of the recommended amount? I'm not dosing further because I don't wanna kill my fish but if somebody gives my the green light I will...?

I am keeping cichlids with my plants, and any newer plants I have, or stem plants in general just get knocked around constantly until they develop good root systems
I would slowly up the ferts though, just to see what would happen. I am running a high light set-up as well and plan on outfitting it with CO2, better flow, and a nice dry fert recipe sooner rather than later (CO2 might have to wait for a tad though)

3 jack dempseys (2m 1f, 1m is ebjd)
1 parrot fish
1 giraffe catfish
1 bala shark
1 featherfin synodontis
1 indo datnoid
1 bristolnose pleco
I'm thinking my giraffe catfish is the culprit I've physically seen him uprooting them. I just don't know if its just him. The bala shark I've also seen nipping plants. I've also seen my featherfin try to eat roots (unconfirmed) or at least eating something that happens to be next to a plant with overgrown roots sticking out of the substrate.
