Looks like some sloughed off dead tissue. Try a little salt to the tank 1tsp per gallon (once only) and iodine to the area with a qtip.

Agree on the salt. Do you have exposed heaters in the Tank?

Looks like a bad scrape as some of the surrounding skin is scratched. Could also be heater burn as noted above.

In either case keep water clean and add salt as directed.

Update is it looking better? This is the first time I have encountered a situation like this

Good thing is looks very superficial. Clean water and salt should clear it up quickly. Iodine will keep it sterile.
