That's sad to hear, one of my first fish as a kid jumped out of his tank, luckily it was a snakehead so he was fine (this was over 20 years ago btw when I was around 10-11)
 
Sorry about the fish. Brings back memories of one of my 20+ year old clown Loaches jumping out during a water change in July. Sadly I didn't notice as there was a lot of stuff around the tank and it had never happened like that before. From that same group, I had one out of the original 5 jump out of the original quarantine tank through a gap in the hood similar to your situation but never during a water change.

Now count all my fish after a water change. I also keep the sides of the tank clear and I try to keep every gap covered in some way.

All we can do is try to learn from the mishap so it doesn't happen again I suppose.
