1 Cory, 4 Cardinal Tetras, 3 Bloodfin Tetras, 2 Angelfish

Yes

Live plants, driftwood, pea gravel bottom.

Not very often b/c of plant roots.

Floats sideways about half the time, gasps for air at the top. Some lose of scales. No bloating or anything.

14. a. Have you treated your fish ahead of diagnosis?
b. What meds were used?

No treatments for fish but tank was being treated for algea with Tetra Algae control
 
Do a 50% water change immediately. Algicides are common fish-killers.

Also, try to get a liquid dropper test kit result for your water. Ammonia, nitrIte, nitrAte. Ask your LFS (Petsmart and Petco can do it also) to do it, and give you the specific numbers that are the results. DO NOT accept "they are fine" because petstores are infamous for giving wrong information on water quality.

How long has the tank been set up? After you do a 50% water change, with temperature matched and dechlorinated water, wait an hour or two, and do another 50% water change. This is the best advice I can give you to help save your fish. Please try to get the test results so we can advise further.

How big is the tank? Do you know the model number of your filter? (Sorry if you have answered these already, it's hard to follow the way it was copy/pasted.)
