sick fish

Have a sick fish.

Symptoms:

fish was rubbing on bottom and rock and noticed two white bulges protruding from beneath scales and some lost scales. I does not look like ich, the bulges are large compared to ich and it seems to be turning into a sore. fish are active and eating it does not look to be damage from a fight. Any ideas.

Started treating with aquarium salt and raised aquarium temp. Ammonia level 0 Nitrate slightly elevated, did a 50% water change just in case is ich but again it doesnt look like it.
