Hey everyone,
A few weeks ago (3) I had to move my GT out of the tank that I kept him in with a female Jack Dempsey (for 8 months or so) because his fins were getting torn up and he wasn't allowing the Jack to eat. I have put him in a smaller tank for the time being hoping that he could start to heal. He has been sulking the entire time I have had him in there. I think it's the first time he has been alone. So three days ago I noticed he was hiding in his cave a lot, which isn't normal because he always greets me and is social. Yesterday I woke up and he was breathing hard, his dorsal and anal fins were pinned, and he seemed restless ans spaced out. He still ate, but didn't show the normal interest he has for food. His poop was normal, thick and brown. Later in the day I noticed he tried to rub/scrape against the substrate. This made me believe it was a parasite issue, so I promptly changed 50% of the water and added FMG. Prior to this I also removed some water so the spray bar would pound down into the water and tested the water parameters.
Today I woke to the same thing- labored breathing and clamped fins except today he isn't moving around the tank, he is just sitting in one spot, oblivious to my presence. He also had some brown stringy poop that changed into white as it was exiting his body and I just observed him attempt to rub again.
This seems like it could me more than one issue going on, but I am not sure. The Jack that he was with does have some health issue that we haven't been able to solve, but it has been three weeks, so that would seem strange to have affected the GT at this point. We also took a tray from that canister to put in his filter to cycle it faster. I believe based on testing it is indeed cycled. The only thing I can come up with is that recently in cleaning tanks I shared a gravel vacuum between the Jack and GT tanks. But Jack isn't acting the same way that GT is.
The results are: Ammonia 0, Nitrite 0, Nitrate 20, pH 7.4, temp 76 inline heater. Used API freshwater test kit. Well water treated with Prime. Water changes 2x/week 30-50%. Gravel bottom vacuumed 1x/week. I have not tested GH and KH lately, last time KH was 19 drops. Recently koi pond was at 14 drops. Usually extremely high due to water source. He has a few large rocks on bottom to make a cave and a few plastic plants. Small 29 gal hospital tank (best we could do). Have not added salt, do not as normal practice. I do have some medicines on hand.
Thank you for any help, I really like him and would like to save him

What is FMG? It could be a bacterial infection, and my first to go medicine is kanaplex since it is readily absorbed through the skin. If you have methylene blue, I would start a giving him at least 30 minute baths (separate container) for a period of 3 days to see if there is signs of improvement, dosing at 1ml per gallon.

Thanks for the reply! FMG is formalin/malachite green which I again used last night. He isn't hiding in this cave today, but mot really moving around either. Still gasping, trying to rubstratch his gills on rocks, and his poop is a white/clear and brown mix. He is also barely eating. I don't have Kanaplex. I also have methylene blue but then noticed it was expired. But I can run out and get these medicines if you think they are best for this situation
Other medicines I have on hand are BiFuran, Lifeguard, Praziquantel, FMG, MG, Clout, MetroPlex

How is the aeration in the tank? Does it have an air stone? Just making sure the FMG is not keeping the oxygen levels too low in the tank.

MB can help with some external bacterial infections. It will also help bring in more oxygen into the fish's bloodstream, helping to boost the immune system and recovery. So I think you should get the kanaplex and MB. If you decide to use kanaplex, make sure that the FMG is completely out of the water. Kanaplex and Bifuran can be mixed into the MB bath. Dose 1ml MB per 1 gallon, and make a new bath the next day. Also make sure that the water for the MB bath has been dechlorinated at least 30 minutes prior to adding MB. Water conditioners can make MB ineffective if dosed too soon. I personally like to wait 1 hour.

I do have an air stone in the tank and I have taken water out each day to reduce the water level below the spray bar to increase the oxygen. I have been letting it go like that for several hours each day before I do a 50% water change and vacuum (I have been assuming his issue has been parasites and wanted to keep it as clean as possible).
I am trying to get pictures now

Sorry about the picture quality. Poop pic is a bit hard to see but there is strands of poop at 12 and 6 o' clock mixed with mucus-like white poop. The pictures of Clovis himself are pretty unremarkable. The pic with the white spot on his head is a water spot.

Do you mean the dark spot on his head? He did freak out the second day I added FMG and flew up into the cover and hit his head. Last night I was only able to find Kanaplex at the store, so I water changed the FMG out of the system and added that. Today he continues to gasp, and is extremely restless, however his fins are no longer clamped. This is the second day he has hasn't ate anything.
