just saw it today yesterday was fine and its with me from 15-20 days with a water change 1-3 days and is eating only turtle food it reject s vegetables

Does the shell feel soft or give at all to gentle pressure? Are any bits sloughing off of the shell? Is there any foul odor coming from those spots? Those would be the signs of shell rot.

It looks to me more like your RES is starting to shed its scutes and there is some water or air in between. This occasionally happens on my turtles, where water gets in between a scute that isn't fully detached. In the short term, this is normal and not a problem, but if the pockets of water remain for a long time, bacterial growth can begin to lead to shell rot. Keep an eye on his scutes, ensure that they are shedding, but do NOT ever force a scute off. It can be very painful and damaging.

Like others have mentioned, ensure that your turtle has a place to fully emerge from the water and dry off, top and bottom. Be sure to have a lamp that provides UVB light and that his diet has enough calcium (cuttle bones are a great source.) I also provide a heat lamp right above the basking spot to aid with digestion and help him dry off fully.
