what size tank is he in and what are you water parameters? that definitely looks like HITH and needs to be corrected right away if those holes are going to heal properly and not scar

He is in a 125 gal and just recently moved him but just recently had water checked and checked out fine. I just did a water change today. What is HITH? So water changes 2x a week for how long?Thanks for all the suggestions.
 
where did you move him from? do you feed feeders? if your water params are good and you have a good water change routine then you will need to medicate to get rid of it. HITH is Hole In The Head disease and is usually 90% of cases caused by poor water quality but is also caused by a parasite so if yours has been housed in good conditions with good food then you will need to treat for it

and what is "checked out fine" what were the actual parameters? and how often do you usually change the water?

Had the water checked? You dont have a test kit yourself? If not, you should probably get one, but the best way to stop/curb it and allow it to heal is , as said above, 50% water changes AT LEAST once a week until its gone but for the next month or so probably 2x a week.

It primarily happens from bad water & diet. Feed him a good pellet (NO feeders, beefheart or other crap) and do the water changes as suggested and it should heal up.

Well the tank made two moves due to loss of a job and a relative was keeping him for us for about 4 months and unfortunately water changes only occurred about once a month, how often do you recommend water changes ( I have 2 large HOBs and only have him which is about 10 inches a 3 inch JD and a 9 inch Pleco in a 125 gal ?). I feed them mostly hikari gold pellets and nightcrawlers,and crikets, no feeders. I took a sample in to lfs and was told the water was fine but I guess i need them to show me the exact parameters. How long will it take to heal this up and how frequent should i do changes to take care of this problem, and will this spread to the other fish? Thanks for all the help.
