spotted grouper rubbed skin?

So, I have the small (5-6") spotted grouper that I recieved for free about a month ago. The orriginal owners of this fish have had it for about 2 years in a 40gal bowfront tank with so much liverock that there was barely room for the fish.
Anyway, he seems to me missing scales on his head, this was like this when I got the fish & they said that it's from him rubbing on the rocks all the time.
Before I introduced it to my own tank (55 which had just been upgraded to a 125) I quarantined him, treated him with pimafix & did a freshwater dip as a precaution, I introduced him to the tank after this was done & he seems pretty content. He eats well, is active & aside from the skin rub areas appears very healthy and alert.
I was just curious if anyone might know what it is. I have seen hole-in-head, but this doesn't look like pits at all, it's just smooth as though the scales are rubbed off. The previous owners said it's been like that for about a year.
Do you think with good care (their tank was pretty filthy) the scales & coloration might come back?

Dang. That is really sad. Could be permanent scarring from HITH.

If it has been that way for a year and doesnt start to improve immediately with appropriate husbandry I would say that the fish will most likely stay that way.

That's too bad. I'm hoping it improves a little, but he's still a cool fish regardless, almost like an Oscar as far as temperament.
I know they get pretty big, but do you think this one is stunded? I would think he'd be bigger than 6" if he's really 2years old.
 
So it appears that the scales ARE growing back, and he is much more active in the tank now that I've done a round of "lifegaurd" aquarium medication due to some sort of wierd lice thing that was making it's rounds on my fish.
