Leave him alone... the weight loss from not eating will get him free...
not 100% on this. thats how my big pleco died. got its head into a hole up to its gills. aperentally they dont got reverse? but was dead and couldnt a been there over 2 hours.

Never leave that fish alone. In this case, destroy the decor to pry the fish open and stop using decors with small holes.

Reverse? They are potentially doing more damage doing this than forward but with a head stuck, the decor must be destroyed anyway. I don't like most decors such as castles for this reason.

Raphs do this ALL the Time in my tanks... I leave them alone and they come out on their own, It's one thing if they are Pinned in...if they are simply stuck (one of their spines will get caught) after a few days or a week they will free themselfs and come out.

I have one spotted that comes out of his cave once every month or so to eat... then jams himself in there again till he's ready to come out again (I've poked, pulled, blown.. air dryed... annoyed and even shaken the crap out of him and couldn't get him out)

they can swim backwards, there actually very powerful and can thrash around quite a bit, either this is not what killed him, or he had his gills caught and could not back out, then died from fighting it. i had a 22" clown knife get caught in the opening of a milk crate, he thrashed around until he ripped his belly open then his tank mates smelled blood and decided to start snacking on him.
