I just noticed while I was doing a water change that my red tail golden arowana had symmetrical holes on both sides of its lower jaw. Some holes are also on the operculum ( gill cover ) but still symmetrical. Is this hole in the head ? What do I do ?

Set-up : 100 gallon tank with no tankmates. I do 20% water change weekly. RTG is around 14 inches and roughly a year old. I use 2 OHFs ( 3 layers per side of the tank with bioballs, green mesh and cotton wool ). purely MP diet.
