Not accepted by everyone, but generally speaking, by the masses.

"crank call"

"I could care less" (if you could care less then why don't you? lol)

"clip" as in a gun clip when they meant a magazine

"decimate" originally means to reduce by 10%, not totally destroy. now it's even in some dictionaries as one of the definitions as totally destroy

people often use "lay" when they should be using "lie" example: I'm going to go lay down. they should say I'm going to go lie down

"I seen" as opposed to "I saw"

"a whole nother"

"intensive purposes"
 
there is no question that can begin with "why didn't"

it's incorrect to start a question with those two words
 
most people call all adhesive bandages "band-aids" and all cotton swabs "q-tips" regardless of the brand
