Thank you all for the comments - Epsom Salts seems to be helping. I think this is a HUGE improvement over laying on his side in one place 'gasping' - he is taking breaths at regular sequence now. I simply could not take a pic of him tin that position. The bulged sides may be smaller. I am keeping my hopes up. Question - how much epsom salt is too much?

The usual recommendation for therapeutic treatment with plain regular Epsom salt is 1 tablespoon per 5 gallon of tank water. It is best to dissolve it first and add to the aquarium over a few hours and watch the fish to be sure it is not in any distress.

If the fish is in distress or appears worse, perform a 50% water change to remove it IF your fish are used to that percentage of water change and your nitrate level is normally low.

The nitrate/nitrite levels are concerning considering your w/c schedule. You may have to go 80% each time. Keep temps at around 78. Cooler temps can aggravate 'clogged up' fish as their metabolism slows.
