Uh-oh: Advice needed please

Here is a picture of my P. humerosa. I've had him since July and all has been well until recently.

Today he is swimming at the surface and has this dent on his forehead. He is otherwise eating well and plump in the body.

Any advice is appreciated,

for the ray to have the dent he must have not been getting enought for since you got him i would give him 2 big feeds twice per day for a few months

Actually, now that I think about it, the way that I deworm rays is to go down to your local vet, get some Praziquantel and some syringes, and inject small amounts of it either directly into your ray, or inject it into some earthworms (which will kill them quickly) and have your ray eat the earthworms. Does the trick every time. Also, slowly bump the temp up to around 85-87, and add one LEVEL tablespoon of kosher salt per five gallons of water.
