What's going on here? I'm going to do a water change now and add prazi pro. Any other suggestions?

Check ur water quality. Include ur Gh and Kh. Start feeding metroplex and dosing salt 1 tbsp per 5g to the tank. Keep water temps 78-80. Would start dipping the fish in methylene blue daily. Feed a good quality pellet. Other will chime in.
I haven't started the water change but thanks so much I completely forgot to test the water. Will do now as I get the water change ready to go.
Do larger water changes, like 90%. How often are undoing water changes? Wha and how often are u feeding? Would recheck the water as ur nitrates are high. Do a water change and stop dosing the prazipro.
It's in a 125 and it's been a month since a water change. Fed about 3 to 4 times a week. All right thanks I'll do another water change. I have 3 HOB filters and one broke so totally my fault for not replacing it quicker.
Yea. Get those water changes going. Twice weekly water changes and reduce ur feeding to every other day.
