Unusual Grouper problem..

So, my saddled grouper has recently developed an unusual problem. He gets these blotch skin marks, and several white pimple looking things on his face. He occasionally pops one gill plate way out to the side while resting, but can move it back to the normal postion if I bother him.
The symptoms come and go. Yesterday this picture was taken, today he looks fine, etc

Er, Sorry about all the algae. Im working on that. I've been using this big reeflight on the tank lately.

Its not ich. The white ''pimples'' are way too big. and the white on his body is actual blotches on the skin.
 
The could be a parasite like lice, some types of viral infections (nothing you can do about those they settle themselves), those are all I can think of off the top of my head, I would suggest close inspection of the "pimples" to see if they are a parasite.
 
I can't really help you, but If you live near Pet Goods in yonkers. Thier is a guy who is the manager of the fish section his name is Anthony. He is a very nice guy and very knowlegeable. In my opinion large water changes may help also you should try placing in urchins. Urchins will clean that tank up and the grouper cant eat them due to thier spines.
