What caused her death?

What could possibly caused it?.

When i saw this i thought it was ammonia poisoning. Then i checked the water parameters all is good. Which make sense to me; If it really was the water issue / ammonia poisoning, everyone else would have stopped eating, but it was only her stop eating since 3 days ago. Please enlighten me.
