What could be causing deaths

So I setup a 40B planted a little while back. I've been losing rummynose tetras and neons and cannot figure out why. Any ideas would be greatly appreciated.

The fish that have died seem to segregate themselves and stop eating until they wither away to damn near nothing. Then they become so weak the slight current in the tank pushes them around until they are dead. No external signs of any injury or illness. Inverts all seem fine, as do most of the fish.

0 ammonia
0 nitrite
10pmm nitrate
Weekly 50 percent water changes

Stock
1 pair apistogramma cacatuoides super red
6 rummynose tetras (2 left)
3 neons (2 left)
1 Dwarf gaurami
5 Amano shrimp
3 nerite snails

I would keep an eye on the Apistogramma even though you stated not noticing injuries my pair is pure evil and chases my Rams constantly. Constant chasing can lead to stress of the Tetra if that is the case.
 
They don't seem to have any pattern to them. I just pulled another neon out that quit eating days ago and went off on his own. I could tell it was near death anyway as it shriveled up to a tiny size. Almost looked like the spine went crooked. I am beginning to think this is an outbreak of neon tetra disease but seems to be affecting the rummynose in the same manner.

I haven't kept these kinds of fish since I was a kid so I am having to relearn the different diseases. I am praying my apistogrammas can't catch this since I paid an arm and a leg for them.
