What is going on with my kels?

I have a 220g with 3 peacock bass. One is an Azul and two are 24K Bahia Kelberi. The largest kelberi over the course of a couple months has developed something on its face. At first it seemed like HITH so i stopped feeding them feeders and have been trying to switch them over to tilapia. The azul took no problem and i can almost hand feed him. The Kels are way less aggressive and dont even hardly go for tilapia. Its been 10 weeks since this developed. It started with pitting of the face. I noticed when i caved and fed feeders it would get noticably worse in the next day or two. So im on week 10 of the kels starving and there are no more pits just a big mark accross its face but it looks like the flesh is being eaten away and there are little black specs which are barely visable to my camera but is alot more visable to the eye. The smaller kel has some real slight marks on his face of what looks like this same thing starting with it. The azul is 100% fine and has shown none of this at all. At this point the kels are getting sunk in bellies and im starting to worry. Its a difficult line here because if i feed them what they will eat this gets worse quick but if i dont they just wont eat.

As for my setup:
2x Fluval FX6 canisters with Substrat Pro bio media and Chemi Pure Blue carbon.
Aquaclear 110 HOB with just the stuff it came with.
I have a Neptune Apex tank controller on my tank as well.

My temp is held at 85-85.5 degrees F
My pH according to my probe sits around 8.25-8.6 depending on variables.
Nitrate sits around 10-40

i do 50% water changes biweekly. I have tried doing 70% weekly and even slowed down and it didnt make a difference to the fish.
I test my water before and like 24 hours after every water change.

Looks like beginning of hith that has progressed a bit. Can try feeding some metronidazole in food and dipping the fish in methylene blue. Maybe some of the other cichlid and pbass owners can offer more.
