What is that on my eels face?

He is eating but less than usually. No rocks no sharp things in tha aquarium... Is he sick?

Could be a fungal infection, the pvc edge looks sharp enough to cut the fish if it had a startled moment and darted into its cave. Add a little salt to the water and pray it heals up.
