What is this??? My betta is disintegrating

I'm treating this with an antibiotic but it's not helping... It's still progressing into his body. What is it and how do I cure it?

Is he in a bowl? If yes, get into tank with heater, no heater? Get a heater. If you have these, what are the water params, and treat for fin rot.
