Hard to tell. How long did you have it? Potentially a nasty bacterial infection or physical injury from something in your tank.
 
I'm at about six months with it.
It just came out of qt for ick two weeks ago with some new additions.
I added a small yellow, porcupine puff, a small bicolor goatfish, an blue griddled angel.

As mentioned above, sounds like a bacterial ulcer or physical injury that got infected. Is there any invert that could of stung this fish?

What did you treat it with in the Qt? How big is your system? Odd it went that fast to me. Maybe the Paramaters when you have them will lead to something.

Copper and garlic food. 65 with 15 in the sump. I was heavily feeding the tank to not shock the system.

Any chance your puffer bit it? They are known to do that especially in cramped quarters. Something like a bite if your nitrates are high could do it. I assume they are since you are heavily stocked for a 65 even if these are all tiny, and you state you were feeding heavily on top of that. Butterflyfish need pristine water, nitrates over 20 can cause strange things to happen. I hope your planning on a bigger tank and soon, your going to need at least 180 gals in the next year or so with that collection, and probably 300 in a few years. Sucks to loose such an awesome fish.

The puffer came in emancipated and only ate two or three ghost shrimp every three to four days in qt. the puffer died when he stopped eating after being introduced.

The upgrade I'm looking for is a 7x2.5
No fish is over 3 inches.

Prams are all good with no2 and no3 under 5ppm ammonia is at 0.
