what is wrong with him?? please help

my spotted salamander which ive had for like 4months is sick

one of his eyes look messed up, and it looks like one side of his chest area is sunken in, and one toe on each front foot (the same toe) is 1/2 gone

i havent treated him anyworse than my tiger sallies

but he was strange, at first he would sit in his water bowl all day, and then after a while he just went under his plant pot and stayed there (never came out)

what is wrong

i dont think his chest was sunken in tho, but yes his eye looks swollen, and the toes, yes
 
How often did you change the substrate and the water? Sonds much like amonia made damage, swich to wet kichen paper for the time being and change daily both the water and the substrate. Also move your sally to the coldest place at your home except the refrigerator, hopefully it will recover
