What is wrong with my clown loach?

Came home, saw this:

Fin rot maybe? The clown is in a 125g with 2 plecos a BGK and other clowns. I bought a 210g for them, but had to move their 125g so the 210g could go where the 125g is... so on Sunday, I moved their 125g and put them back in it which I am sure caused some stress - but I haven't moved filters or anything over yet so the BB should be fine.

Finrot usually starts at the ends of the fins and makes it way down to the base so I don't think it's finrot
 
I am starting to lean towards some kind of injury...

Last night I did a 50% WC and dosed w/ Melafix. Fish was still acting fine this morning, ate its breakfast and everything, still had the mark though -- no one else is showing any other signs. I am going to do daily water changes for the next few days and continue with the Melafix --- I don't know what else to do... ?
 
Is it starting to go away? he could of hit something and got hurt. Does it stick out or is just a spot on the fish
 
i saw this before at petsmart there clown loaches had looked like this ghostly white coating and they didn`t swim much maybe bad water condintions ? This happens to my eels idk maybe because both are sensitive to water conditions
