what kind of angelfish is this? please help!!

Looks like a Silver angel to me. Phetrophyllum Scalare.
 
Like 99% of the angels in the hobby, it is a hybrid. I would say this particular one is probably the result of crossing a black angel with a silver angel, but keep in mind that "black" and "silver" aren't actual species... they're just trade names for two of the many color variants that have been produced. There are also koi angels, blue angels, smoky angels, and many more. All of these are domesticated angels that have resulted from decades of outbreeding, hybridizing, and line breeding. They're often erroneously referred to as "Pterophyllum scalare," but the only way you can get a real P. scalare is by obtaining them from a reputable breeder, or by obtaining wild-caught individuals.
 
It's either a black lace or hybrid black. Both are dark angelfish that still show their bars underneath. DD blacks (double darks) are usually so black that the bars can't be seen, but even then that's not always true. Then you have black ghosts which carry a stripeless gene and won't show their bars, but this is obviously not the case.

The fins are short because they either rotted off or were chewed off by tankmates, and are now growing back. I can see the damage on them in the last two pictures. This is not normal and hopefully they will grow back to full size if they didn't scar over due to poor water quality when the fish was first injured.
 
When I got him he looked pretty bad. He is already 5 times better than when i got him.

I do water changes, water checks i also keep a log of water quality every time i check.

I also have 4 other angelfish that are immaculate.
I am sure one is a black lace.
2 are koi angels
And the other i dont know what she is.
 
The koi are a mateing and have a space to do there thing without disrupting the rest of the tank.
