Looks like it could be hole in the head if so then frequent water changes should hopefully clear it up id say use salt but cats can be sensitive to it

Doesn't look like HITH to me. It's red and looks purulent. I say bacterial. Just keep the water clean so he can beat it himself. If you really have no ammonia, no nitrites and very low nitrates, this should heal on its own. Looks like he bumped his head on something.
