Looks like a prolapse. Don't feed for a week or until it fixes itself. Add dissolved epsom salt about 1 tablespoon per 5 gallons to reduce any internal swelling. Keep the epsom salt concentration for at least 3 days, continue if you see progress in the prolapse slowly going away.
