Wow, he's really grown since I saw him as a fingerling. Never would have expected him to survive. The head is basically an extra gill. When I saw it last, the head and eyes didn't move. Nor did it try to eat with the extra mouth.

Genetic mutation. Most fish that have mutations such as this rarely live past fingerlings. Still looks creepy to me @_@
