Whats My Problem? Plant Leaves Developing Holes!

Ive been doing a bunch of reading on the effects of different deficiencies that cause plants to die or suffer, but almost everywhere I look they tell me different reasons for different things. I'd really like a straight answer, if possible that is on what my particular issue is, so sorry for posting another one of these threads.
Here are the conditions:
75G
4 48" 40watt fluorescent bulbs, two Hagen Aqua-glo, one Hagen Flora-glo and one reptile bulb of some sort, 12 hours a day
1.5" of Flourite gravel
30% W/C every two weeks
Ive been using the recommended amount of Kent Pro-Plant
Houses Discus & angels plus a few others,
Tank has been established for years, never had any issues with fish, thought I'd start trying live plants and this is what I get:

have yet to see a snail and i spend a looooot of time lookin in that tank, perhaps that type of otto in the 2nd picture? That or ghost shrimp? Also have a snowball pleco
 
the guys about 2.5", 3 tops. you really think its the culprit?
 
Those sword plants when you buy them and they have those oval round leafs have been growing out of the water. They cultivate them semi-submerged. Those leafs are now submerged and dying off. The new leafs that will grow will be longer more sword like in shape. I would just trim all those leaf off so the plant doesn't have to use energy on those old leafs and use all its energy growing new leafs. Looking at the whole plant it seems to be kinda yellow like they need more Iron supplement.
 
okay, so im hearing that it may be a lack of iron or potassium. That or the fact that they are now being submerged and dieing off. Yes I do fert with that kent proplant stuff. Do you all suggest i go out and find a iron or potassium specific fert?
oh, someone told me the other day that my lighting wasnt sufficient, do you think thats true?

Could be potassium deficiency. Your lighting is probably fine, the plants would be weak and spindly if they were lacking light. I add iron daily and potassium sulfate every other day.
