I removed one of three from the tank because he had an eye injury and his find were getting nipped at pretty bad. As soon as i took him out i noticed that one of the other ones went inside of a hallowed out head that i use as a cave for my pleco. This was two days ago and i have not noticed him come out of it since then. when i shine a flashlight inside i can see that it is in there floating normal and seems to be healthy, i havent even seen it come out to it. It is only about 3 inches, could it have spawned eggs already?

don't worry, you are not a grandparent yet.
it probably is stressed somehow, as they are being so small.
the other reason is that the one swimming around has been picking on this one so it went into hiding and wouldn't come out.
how's the one eye one doing? if it's doing better then you should put it back in the tank and remove the "cave" in the mean while so they can reform the group.
what size is your tank?

i am not sure about the one with the eye. i put him in a 10 g and he swims around like normal then when i come by the tank he lays between a piece of slate and the bottom of the tank. also his fin dont seem to be forming back yet but i only started to feed him in there yesterday so im sure once he starts eating regular again they will come back much faster. the tank is a 55 g so they have plenty of room right now.

it probably takes about a week or so for the fins to grow back. as long as there's no signs of infection you can keep it in 55g with the rest of them, that's what I did with my caribe when it lost one eye. I left it in the 180g with the rest of the shoal, just added some salt to the tank and keep an close eye. at that small as long as there's enough food they should be fine. of course you can keep it in the 10g if you worry that it'll get picked on.

One of my eleven piranha lost its eye during one of its feeding frenzy .. I left it in the same tank but just made sure its getting some food .. eventually it got used to having one eye and now its doing fine .. raising piranha you will see fins or bits of body getting bitten off .. it will eventually heal up .. I find it that if you keep up with their on schedule feeding and giving enough food .. you will reduce the fins/body biting

BTW .. reason I didn't take out the injured piranha because I assume that if I take it out for a long period of time .. the other ten would reform their little group and when I put the injured one back it will be an outsider and get killed but I can be wrong

what you feared was very possible, that's why if it's not very serious injury better off leave it in the main tank.
I had two wild reds got part of stomach bitten off and I left them in the main tank. two weeks later, they are good as new.
