What's wrong with this synspilum?

It looks all swollen in the lips and now has what looks like pimples in front of both eyes and 2 under its chin. Both sides looks exactly alike. Any help would be great.

It belongs to my brother but I believe he just feeds omega one pellets.

I would do a water change and try some maracyn, look bacterial to me.
