We've been having a lot of issues with the tank and fish keep dying, including a pretty big sunshine pleco. Rest In Peace, buddy. Anyways, our LFS has tested our water multiple times and said things look good but fish keep dying. We are done buying fish and just really wanna prevent the three beacon from dying too.
 
Get methylene blue, and place in a bath for 30 minutes; dosing at 1ml per gallon. Do this for 5 -7 days. Throw the bath water out when done.

What's the reading for the liquid nitrite test? What's the reading for nitrate? I don't put much stock in strips since they tend to be inaccurate.

How long has this tank been set up? How often was the water changed, and how much water? How large is the tank?
 
is it both eyes or just one? looks like an injury to me
 
If it is an injury, use mela-fix, it's always worked for me no problem.
