Im not sure if he knocked a scale loose or this is something else. The other day he got spooke and went nuts but i dont remember if the patch was there or not. Its on the top of his head. Should i be concerned ? Add salt ?

that looks like missing scales to me. my grass pickerel constantly has that from chasing food around the output from the sump. I'd add a little salt and keep an eye on it, most likely nothing.
