White, yellowish and black spots

I have three Geophagus that were purchased with some type of Disease, bacteria or parasite. They started with what looked like yellowish or white nodules only on their fins. Fish were treated at 86 degrees with ich chemicals for two weeks with no change and 2 further weeks at the same temperature with salt. Neither route in a 20 gallon quarantine had any effect and locations of the spots slowly have grown on two of the specimens. There are also now some spots that appear black and some look to be imbedded into the muscles of the fish on the actual bodies. Appetite of the fish are fine but they do flash periodically. To reduce stress the fish were placed into a 75 gallon tank by themselves and ich treatment has been stopped till I can actually determine what is going on. I thought that yesterday when I was reading up on it. That’s why I made sure to reacclimatize them to their preferable temperature and get them in a less stressful environment (75 gallon). Only thing that strikes me as odd are the black modules... maybe they are the fish fighting the viral infection and healing (dead blood cells)?
 
Also possibly what looks like a worm from one of the gills, Gill flukes? I’ll try and get a pic of that.

I don’t see it but a round of prazipro with repeat dosing will help. Black spots could be black spit disease but could be normal coloring. The prazipro should help with that too. If u can get better pics of the worm in the gills it’ll help. Like I said I don’t see it.
