The type is called an HQI fixture. You can compare it to other HQI fixtures, but it seems like squirtle got the brand for ya.
As for the t5's, there are 2 common ways that they can be removed. The easy ones simply twist out like a t8, and the more difficult ones (more common in retrofit fixtures) is to have 2 black end caps which must be unscrewed from the base and then slid off the bulb. Those are a pain as the bulbs sometimes melt the plastic a little bit, making them sticky and hard to remove.
To access the bulbs the protective plastic/glass plate will typically slide out of most fixtures. Often times with HQI fixtures there are a couple of screws on one side. Removing them will allow you to remove the metal bracket holding the plastic/glass plates in place.
