Wolffish damaged scales

Can anyone tell me if this looks like a scrape or an infection. Literally happened over night and the only thing different is that I added a clown knife (smaller than the wolf) and was wondering if it was a scale eater or something... But did not find any info to back that theory up..at this point going to treat with some melafix, but any suggestions would be greatly appreciated. I don't want to lose my fish
 
Looks a little more than a scape. Would keep the water clean. Add a little salt, 1 tsp per g. Watch and see how it progresses. Any hard decor in the tank?
 
Once a week, 80% water change. Had it since it was the size of 1/3 a hotdog, now he's like a 9 inch fat zucchini/pickle. Never had any problems, all the other fish are OK. I did space out while refilling the water from tap and forgot to add prime until the tank was just about full. Could it be an ammonia burn from sitting next to the hose? Usually the prime is already in the water treating the incoming water as it mixes. I had thought the clown knife maybe was picking at him, but I don't think it could have done that without getting his head ripped off, let alone in just one night. I will add salt and take out a piece of coral
 
Weird pattern of injury. But should heal up. Don’t think it’s an ammonia burn.
