Midlands, England.
Water perameters are all good. 50% water change at least every week. Had for some time but got better, now its worse. The oscar doesnt move much really just sits on the bottom and never really eats. (usually feed hikari pellets.)

Any idea what this is & maybe some help with treatment would be appreciated.

....although looking at the gill area, and thinking it over...especially with the pic of him laying on the bottom, it could be fluke parasites. How long has this been going on? If it is flukes, it's going to be tough to bring him back from the looks of it.

its been going on for months now. I'm not sure if an oscar would normally just sit on the bottom like that, i really doubt it. he is in the same place now as when i took the picture about 7 hours ago, he just sits there all day.

Got to a better screen, upon closer look IMHO....i believe your fish has an advanced case of hole in the head, their is white mucus coming from the gills. this disease is caused by poor water quality and possibly poor diet and nutrition. You need to call a vet and tell them you need Metronidazole to treat this. You need to do daily water changes. The water needs to be pristine. Even if he recovers he may still have scars, no white spots though. This disease is common in oscars as they seem sensitive to the disease. Regardless consulting a vet or local marine biologist via phone might help. We are the Internet. No one should trust us.

That's one miserable oscar. I don't understand seeing this "going on for months" without getting help.
?

I had two oscars both come down with hole in the head at the same time. I did all the water change and remove carbon and a change to chiclid pelets. 2 weaks in no improvement on a random sad google serch to find more info I came acrost a old text only forum from the late 90's In a major artical they made a bold printed state ment that Hole in the head is the same idea as scurvey in humans and that it is a lack of vitamin D in omnivorus fish. I went out found the highest stated vitamin d fish food i could find locally and started to feed them. The dominate larger oscar is alive today and now eats an omnivor diet. The smaller one half his size died in a quarantien tank. http://www.youtube.com/watch?v=lIwdZ3mxwf4 see 1 minute in on the video
