Many stores will test your water for free. Take a sample out to do so.

Then if in doubt about anything I find a good old water change never hurts.

Could be many factors. I was leaning toward water quality issues like high ammonia, nitrates, or nitrites, maybe an uncycled tank. Way too many variables here.

How about a bit of info for us.
Lets start with:
Tank size?
Number of tankmates, the types and their sizes?
Any aggression between fish?
Was this bass newly acquired or have you had it a while?

Yeah it's in pretty bad shape and I'm with D in wondering about the condition of the water.It also looks as if it hasn't had much to eat...
 
water changes with salt. I bet ur snook beat it up!
