I have a 125 gallon which houses 8 tiger barbs, 6 black skirt tetras, 1 Senegal bichir, 1 African knifefish, 1 electric blue acara, 1 opaline gourami, and 3 syno catfish.

0ppm ammonia
0ppm nitrite
20ppm nitrate

The only fish that are rubbing/thrashing their bodies are a few black skirt tetras and a couple of tiger barbs.

I don't see any sign of ich and have dealt with ich several times.

Anything else that could be the cause? I do weekly water changes.

Appetite is normal. Should I be proactive and get some meds?

Fish flash occasionally and would only treat if it is consistent. Are they flashing on the substrate?

About 30-35%
I've been in this hobby for a long time, and the one thing I've learned and can't emphasize enough is water changes. The bigger, the more often.............the better! If I had the time and/or dedication, I would do 50% daily. But, with a regular, busy life, I settle for +50% twice a week. Since going to this schedule, fish health issues have been really a non-issue for me. Therefore, I'd recommend increasing your w/c to at least 50% once a week...but, more would be even better.

Thanks for the advice. HA I was just thinking to myself how I dont have time for the weekly water changes but I force myself. I'll up it to 50% on the water changes and will stick to a strict weekly schedule.
