Head in the hole disease

there a visible hole in the forehead of my flower horn fish and I am concerned about it being related to the hole in the head disease. I am attaching pictures for reference.

Hith would be small pit like holes from the pic not positive that is what it is. How hard is your tap water and what is your water change routine. Is it weekly or every 2 weeks or monthly?
 
High nitrate level is the biggest culprit when it comes to HITH. Some species are from low ph water in the wild and hard water would make them very susceptible.
 
Can you expand on this or provide some literature for me to read? Why would the above situation make the fish more susceptible?


In the Blackwater of the Amazon River fish will not get the disease do to low ph and the river is always be replenished with fresh water . Amazon river high nitrate would not be a factor.

I am not aware about the hardness of the water ... I change the water once a month .. is there any solution for this

I suggest changing water once a week most do 50% and a few members do fin level wc's. I would also recommend picking up a API test kit . The kit will allow for you to test ammonia,nitrite,nitrate, and ph. If possible take more pics like stated I am not sure that is HITH disease.

Do at least 25% a week water change that is probably the problem. Do not overfeed this can also lead to hole in the head as well as poor quality food.
 
I know with oscar keeping, I've never seen or read of case where this issue wasn't accompanied by a tank with very high nitrates due to lack of water changes. This is not to say HITH is caused by high nitrates, but it does seem to go away and heal once a more aggressive water change schedule is adopted.
