What’s going on with my flowerhorn?

I’m concerned. She was doing great the past few weeks no problems. Started acting weird so I checked water. Water is 6ph 0 ammonia 0 nitrite was 5ppm nitrate but did 30% water change still acting weird, no interest in food and staying around the corners. No visible problems with swimming but just doesn’t want to move. Has gone extreamly white to the point where it looks like a different fish and have developed some weird stuff on her eyes the past couple days. Doesn’t seem to be ich as there’s no other spots on her body. She has a bit of color in these pictures but her worse point was when she was solid white with just her black stripes.

Well if the heater is broke and over heating or not heating the tank the fish could be stressing out..also that thing in the eye looks like a piece of your substrate or a scratch that got infected

The fish could flash and scrape it's eye.. any tankmates?
Had her about a month or so. She brushes her sides on the substrate every now and then but not often. Maybe 1 time a week I see her do it. But she doesn’t seem to flick her fins and she only does it once then goes back to normal.

She also gets startled extreamly easy if I walk up to fast. Sometimes just going across the rank sometimes she will bump heater or glass or filter intakes. But never overly bad or worse than she ever has. It hasn’t caused a problem before.

That's all I got really.. hopefully someone else helps out .. the eye really looks like substrate to me
Thanks for your help. Do you think if it’s a scratch that got infected would lead to the lethargic behaviour, all the faded color and no eating?

Looks like a very small tank, no cover, so it will likely be very stressed from being so exposed. The very bright white sand would also encourage it to feel stressed and it might be fading to try to match it a bit.
