So this has been on and off for a little while (3 months) but it comes and goes, just wanted someone to confirm what it is.
looks like fungal but severum seems to be fine eating like normal and water params are perfect

Could be viral lymphocystis. What do you think duanes duanes ?

whats the best way to treat it
It’s viral. No treatment. Supportive like clean water and make sure it eats. Can spread to other fish if stressed.

those are the only spots, one might pop up on the other side but thats as bad as it gets
I could be wrong but I don't think lymphocystis, then. I've had a few fish that get the occasional, odd 'pimple' or two from time to time, cured by maintaining good clean water and adding a little salt and/or Melafix-- so what I've had wasn't lymphocystis. Lymphocystis typically spreads or grows into a larger mass.

Also regarding lymphocystis, not that it's foolproof, but I've heard of people using Usnea with success. My understanding is different species can get different versions of the virus, so as far as that goes what legitimately works in one case may not work in another. Other ways I've seen in science literature to kill the virus involve temperature and high or low pH, like 3 or 11 pH, so not at levels most fish can tolerate.
