I am going to say Fake- being that I own koi- This is a male (look at the pointed type fins) Females are rounded- Anyways, male koi are more torpedo shaped, where as the females get the rounded bellies.

I have a Koi that is almost 30", and I can not hold it out like that without struggling from it's weight.

It is wasn't so large, then there may be a chance that you hold it like that, as I have noticed with my fish that the large koi seem more secure and don't freak as much.

Not to mention, look at how he is holding the koi.. There is no way he would be able to hold it with all the slime coat- Koi are slippery!

Also, his right arm has no water reflection. All the reflection if stretched outwards towards the bottom of the pic, him arm- shows no reflection.
 
Google large carp and you'll see many examples. The one in the OP happens to be brightly colored but still, natural size. Examples from google below.

Based on it's color, I would say it was bred as a koi, and not a common carp- carp bred as koi don't get as large for some reason..

Look at the size of the guys head. ANd the size of the fishes head for reference. That is a huge carp. I have caught a few good size ones in the connecticut river nothing that big thow thats a monster
 
To those who think it's real, zoom in to 400% (assuming your web browser allows you to) and look at the picture. It's pretty obvious that the pic has been messed with if you look at the guy's left hand and arm...
 
Fake.. if you also look at the reference pics.. the Carp can get that size.. but if you look at the placement.. the guys hands aren't where they would need to be to support the fishes weight. I'm 99% sure that was a photoshop.

This is what I was going to point out. The reference pictures, the guys are almost "cradling" the fish, whereas the first picture, he's holding it pretty much upright.
