Not enough lighting, especially for a tall tank. That's why algae grew when you put flourish in it, it provided nutrients to the algae.

Depends, he didn't specify what type of Flourish he used, if it was Flourish Excel (and it probably was) then that should've killed the algae. Seachem has all but officially said that Excel is an effective Algaecide.
What color does the plant turn before it dies/when it's dying? Color or leaves as well as what TYPE of leaf growth is a good indicator of what is deficient in the tank. If they're yellowing you're low on iron, etc.
