Once upon a time, I saw a thread on MFK of a guy that gave his arowana the gill curl surgery without really knowing what it looks like. All he had were after photos. I just took this video of my jardini that has a very pronounce case of gill curl. I was going to do the surgery on it but it hasn't hinder his living yet. Once it does, I'll perform. Until then, I'm not going to risk his life. Here is a video for reference just so that you will know for sure for future reference. You may want to sticky this.

I think I would try improving the water quality and see if it don't improve. Last May I adopted a large silver with gill curl after a month in quality water conditions the GC was gone.
 
Id say it was bad water quality and definitely space, but in this case, it most likely isnt space. Do a bit more WC and maybe add an airstone, it looks like its having a hard time breathing? Id suggest you dont try the surgery, you might harm it more than do it good.
