Just noticed this on my silver aro is this anchor worm? The fish is 20”+ long. Tank mates are pacu giant gourami and Florida gar.
Any recommendations. Do I have to remove it from the fish.

If it’s just one then pulling it off the body with tweezers, fine needle nose pliers or needle holders for sutures will be enough. Watch to see if any occur after removal.
 
I would recommend manual removal if at all possible.

Because these adult parasites resist normal chemical treatment, physical removal may be the best course of action.
After physical removal, I treated the tank with 3 to 3.5 ppt (parts per thousand) salt, as the immature parasites are killed by that salinity (not adults)
To achieve that salinity I use 3lbs + salt per 100 gallons, or 18 gallons of straight sea water in a 180 gallon tank.
Since the tweezer removal, and salt treatment they have not returned, its been almost 2 years.
 
**update**
Pulled him from the tank on a towel with wet paper towels on top. Had him cover for a few moments while he struggled initially. When I looked for the worm it was gone. Not sure if it was an anchor worm or not what ever it was is gone. Lights are off keeping the room quiet. Should I add anything to the water. API Stress coat? Salt? It Was recommended to treat with copper from a few others. What do you guys think?

Leave alone and watch for any more stragglers if develops.
