I don't think it would be wise to take it to a vet seeing how small the turtle is. There has been a ban to import this species to the USA for sometime now.
 
Well ya unless youwanna take a chance on him dying? A responsible pet owner will take there animals to the vet when they are sick.... it could be down in his lungs as well?

This is not a big deal, ...yet. I see the infection but no froth exudate from the nostrils. The infection appears to be topical at this time.

Keep the temp at 84-85F. Add sea mix salt to light brackish levels (spec. grav: 1.002-1.004). Add Maroxy at full dose (this med is safe enough for treating fish eggs). Then, just wait for everything to work. Continue treatment for a week after the last trace of fungal infection has disappeared.
Then, water change and maintain temp at 82F-84F for this species. Also keep the water slightly brackish and alkaline (with bulk limestone and/or aragonite).

This species will occassionally slough the surface of their leathery carapace. Don't freak out if you see this. It's their way of replacing a damaged surface layer.

my experiece keep frt from 2" to 10" is open wound, because when your frt is not the same size or have more than 1 male, ussualy male more aggresive to bite other frt, the most aquatic problem is fungus. but your frt nose not seem like fungus disease. it like open skind wound.
suggest:
if open skind wound, can be treat using repti wound healing aid from zoomed

get off your frt from the tank put into a clean dried basket, take cotton buds give repti wound healing aid, give to your frt nose, keep 2-5 minute until the medicine dried. put back your frt into tank. repeat this treatment as 2-3 times a day. stop this treatment if your frt nose already recover. if there is no change in 14 days bring to vet.

for fungus disease, i using methylene blue from fish vet, yes fish medication can be used for aquatic turtle. the treatment is same like i mention before.
