Totally agree with Petspoiler, if you test the pH of your tap and it's 7.6, and then test your tank and it's 5.5, then you're not doing enough water changes.(each 10th in value is a 10 fold difference in acidity)
That kind of drastic difference can put enough stress on a new fish to kill it.
Or if the nitrate in your tank is 200ppm, you're not doing enough water changes.
.......and so forth.
 
You should refer to A-Z Garden Pond information section on hydra-aqua.com or use Hydra Chloramine-T which is an effective fish medicine.
