how long have you had the fish? what size tank and parameters? tankmates
levamisole and prazi are good to cover most internal parasitic worms
if he is not eating I would try metro in the water for hexamita and spironucleus. Metro works better when eaten but that isn't an option for you right now

tank size was 55 gallon, but relocated to 10 gallon for easier treatment. Had the fish for over 6 months. no tankmates.
